Course Substitution Appeal Form

Overview

A qualified student with a disability under ADA and Section 504 has a right to request a course substitution per State Rule 6A-10.041 ().

If a student is dissatisfied with a decision of the Course Substitution Committee, an appeal may be made to the Provost and Vice President for Academic Affairs within 15 days of receiving written notification of the denial. The Provost and Vice President for Academic Affairs shall render a decision in writing to the student within 15 days after receiving the appeal. The decision of the Provost and Vice President for Academic Affairs is final.
