Offsite Test Request

Request to take tests off-campus

Please complete the Offsite Test Request form below. This form is for students requesting to take a test at an offsite location with a proctor.

You should choose a testing site from the website and get the name and contact information for the proctor for that site. You will include that information below. The Placement and Assessment Center staff will contact the site you choose and provide them with a code they will need to administer the test to you. It could take up to 24 hours for them to receive the information they need. Please contact them for verification before going to take the test.
